Dumping the entire conversation on the reviewer is not a reliable handoff.<\/p>\n<h2>Error propagation should preserve meaning<\/h2>\n<p>In multi-step systems, an upstream failure can be distorted as it moves through agents and tools. Once that provenance disappears, later review becomes difficult because the final text no longer shows which evidence supported which statement.<\/p>\n<p>Preserve source identifiers across handoffs. When a search agent returns findings, include the supporting references. When a synthesis agent combines findings, keep those references associated with the claims they support.<\/p>\n<p>This principle also applies to tool data. Create cases where relevant information appears early in a long session, where tool results conflict, where an upstream error occurs, and where the correct behavior is escalation.<\/p>\n<p>Check whether the system preserves the right fact, drops stale material, communicates uncertainty, and keeps source references intact.
